We are seeking a highly skilled and experienced CHC Nurse Assessor to join our team at NHS North East London. As a key member of our Continuing Healthcare team, you will play a vital role in delivering high-quality assessments and care to our patients.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop personalized care plans.
- Work collaboratively with multidisciplinary teams to ensure seamless care delivery.
- Coordinate and facilitate patient reviews, ensuring timely and effective decision-making.
- Support the development of care pathways and services to maximize patient outcomes.
- Monitor and report on patient progress, identifying areas for improvement and implementing changes as needed.
- Registered RGN with current NMC registration.
- Experience of working in a healthcare environment, with a strong understanding of the public sector.
- Knowledge of project principles, techniques, and tools, such as Prince 2 Foundation and Microsoft Project.
- Strong working knowledge of the Continuing Healthcare National Framework 2022 and the Health & Care Act 2022.
