CHC Nurse Assessor

The post holder will work within the Continuing Healthcare team, delivering optimum holistic CHC assessments in line with good practice and the DoH's National Continuing Healthcare guidelines. They will coordinate relevant assessments and complete required documentation in preparation for presentation to the appropriate CHC Validation team.
Main Duties- Facilitate multi-disciplinary assessments and patient reviews in line with standard operating policy.
- Support the Continuing Healthcare Team leaders and the wider team to ensure the workstream is planned and managed effectively.
- Manage cases as directed by Clinical Team, ensuring Continuing Healthcare policy is followed and services are delivered to maximise patient support.
- Identify and support the arrangement of services required to meet the needs of individuals who qualify for NHS Continuing Healthcare.
- Ensure data systems are kept up to date and maintained in line with standard operating procedures.
- Participate in ongoing professional development and clinical supervision in line with NMC guidelines.
- Ensure compliance with all statutory and mandatory training.
- Ensure revalidation of professional registration is maintained.
- Provide cover for other Nurse Assessors as required.
- Ensure communication remains professional and appropriate at all times.
- Support the Continuing Healthcare department in working towards national performance indicators and business objectives.
