About the Role

Reading Borough Council is seeking a dedicated and experienced Occupational Therapist to join our LDA Team. We are committed to improving the well-being of our community, and we believe that collaborative efforts with colleagues, healthcare professionals, voluntary organizations, and education partners are essential to achieving optimal outcomes for individuals under our care.

As an Occupational Therapist, you will play a vital role in supporting individuals with learning disabilities and neurodiversity across various age groups. Your primary focus will be on providing timely transitions for Care Act eligible 14- to 18-year-olds into adulthood and ensuring that all adults with a learning disability and/or autism receive appropriate ASC support under the Care Act eligibility criteria.

Key Responsibilities

  • Promote independence and successful outcomes for young and older adults through individualized support and interventions.
  • Embed an enablement focus, assisting individuals in remaining in their own homes and maximizing their independence.
  • Collaborate with families to ensure they understand the role and remit of ASC for eligible LDA adults.
  • Develop strong relationships with providers, community services, children's services, schools, and colleges.
  • Contribute to a high-performing, efficient, supportive, and outcome-focused team that embodies the principles of TEAM Reading.
  • Utilize a strengths-based approach and your unique OT skill set to support individuals in finding solutions, promoting well-being, and engaging in healthy occupations through various interventions, including equipment, adaptations, manual handling, goal planning, and close collaboration with care providers, Housing Services, and Health colleagues.

Essential Requirements

  • A degree in Occupational Therapy
  • Registered with HCPC
  • A good understanding of the Care Act 2014 and its application, including Adult Safeguarding
