Job Title: Community Mental Health Practitioner

About the Role:

Livewell Southwest is seeking a skilled and compassionate Community Mental Health Practitioner to join our team. As a Community Mental Health Practitioner, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities:

  • Conduct comprehensive mental health assessments and develop personalized care plans
  • Provide evidence-based interventions and therapies to promote recovery and well-being
  • Work collaboratively with patients, families, and carers to ensure seamless care coordination
  • Develop and maintain effective relationships with community partners and stakeholders
  • Participate in clinical supervision and peer review to ensure high-quality care

Requirements:

  • Registered Mental Health Nurse (RMN) or equivalent qualification
  • Significant post-registration experience in community mental health
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a multidisciplinary team
  • Knowledge of mental health legislation and policies
