About the Role

We are seeking a highly skilled and compassionate Care Specialist to join our team in Radstock. As a key member of our support team, you will work closely with Tom, a gentleman in his late 40s, who requires assistance with daily living activities due to his learning disability and autism.

The successful candidate will be responsible for supporting Tom's independence, enabling him to live his best life according to his preferences. This includes accompanying him on outings, managing his medication, and ensuring his physical and emotional well-being.

You will work collaboratively with Tom's existing support network, including healthcare professionals, to develop and implement a personalized support plan that meets his unique needs. Your expertise and compassion will make a significant difference in Tom's life, empowering him to achieve his goals and aspirations.

Key Responsibilities
  • Support Tom with daily living activities, including personal care and medication management
  • Accompany Tom on outings and engage him in activities that promote his physical and emotional well-being
  • Collaborate with Tom's support network to develop and implement a personalized support plan
  • Monitor and report any changes in Tom's condition or behavior to relevant healthcare professionals
Requirements

To be successful in this role, you will require:

  • A strong background in care and support work, preferably with experience working with individuals with learning disabilities and autism
  • Excellent communication and interpersonal skills
  • A patient and empathetic approach when working with vulnerable individuals
  • The ability to maintain confidentiality and handle sensitive information
  • A valid UK driving license, as some shifts may require use of a vehicle
