Learning Disability Mental Health Associate

We are looking for a highly motivated and compassionate individual to fill the role of Community Learning Disability Associate Practitioner. As part of our community transformation initiative, you will be working closely with TEWV & Tees Valley Primary Care networks (PCNs) to provide exceptional care to individuals with learning disabilities.
The successful candidate will have a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent. They will be able to travel independently across the locality and have access to a suitable vehicle for business purposes.
You will be responsible for engaging patients on the SMI register for physical health investigations, using an assertive outreach approach. You will also work closely with colleagues to provide holistic care to individuals with learning disabilities.
