**Job Description:**

Hunter AHP Resourcing is a leading recruitment agency specializing in mental health professionals. We are currently seeking a highly skilled Community Support Specialist to join our Adult Community Mental Health Team (CMHT) in the Reading area.

Responsibilities:

  • Caseload management: Develop and implement tailored care plans for up to 30 individuals with diverse mental health needs.
  • Care coordination: Provide holistic care coordination, working closely with service users, families, and multidisciplinary teams to deliver person-centered support.
  • Diverse cases: Support individuals with varying severities of mental health conditions, promoting recovery and resilience through targeted interventions.
  • System expertise: Utilize CareNotes effectively to document and track care provided.
  • Autonomy and teamwork: Work independently and collaboratively within a supportive team environment.

Requirements and Benefits:

  • Qualifications: Registered Mental Health Nurse (CPN) or qualified Social Worker with valid professional registration.
  • Experience: Proven experience managing caseloads within an Adult CMHT or equivalent setting.
  • Skills: Strong communication, organizational, and problem-solving abilities.
  • IT proficiency: Familiarity with CareNotes or similar electronic record systems.
  • Driving: Must have access to a car and hold a valid driver's license.
  • Flexibility: Some part-remote working may be possible, subject to discussion with management.

Salary:

£25.38 - £27 per hour (Umbrella)
