Job SummaryWe are looking for an Advanced Nurse Practitioner who is passionate about delivering exceptional care and thrives in a dynamic healthcare setting. You will work collaboratively with our multi-disciplinary team to provide safe, high-quality care and support the overall health and well-being of our patients.
Main Responsibilities- Collaborate with the healthcare team to deliver patient-centered care and support the overall health and well-being of our patients.
- Conduct thorough history-taking, physical examinations, and use critical thinking in clinical decision-making to establish a diagnosis and develop a comprehensive management plan.
- Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatments into individualized care plans.
- Provide holistic, patient-centered care by developing ongoing support and care plans for patients and their families, with a focus on prevention, self-care, and overall health promotion.
- Prescribe and review medications based on the latest evidence-based practices, national and practice protocols, and within the scope of your qualified practice.
- Coordinate patient care plans, ensuring patients and their families are actively involved in the decision-making process.
- Request and interpret diagnostic investigations, such as x-rays, blood tests, and scans, within agreed protocols, to support accurate diagnosis and treatment.
- Maintain accurate and up-to-date patient records in accordance with NMC guidelines and other relevant standards, ensuring patient safety and continuity of care.
- Prioritize and triage patients' health problems, intervening appropriately in complex, urgent, or emergency situations, including initiating effective emergency care when needed.
Great North Medical Group is a leading primary care provider operating from two key sites: Chestnut Avenue in Carcroft and Princess Street in Woodlands. With a dedicated team of 9 General Practitioners, 2 Advanced Nurse Practitioners, 7 Nurses, and 4 Healthcare Assistants, the practice serves a patient population of approximately 17,000, making it one of the largest medical practices in Doncaster.
