About the Role:
Sugar Maple Health is seeking a skilled Physical Therapist to join our team in providing high-quality care to geriatric patients in their independent homes or within senior living/assisted living facilities.
Key Responsibilities:
- Deliver comprehensive physical therapy services to geriatric patients, focusing on rehabilitation and maintenance of functional abilities.
- Conduct thorough assessments and develop individualized treatment plans to address patients' needs and goals.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on disease management, prevention, and wellness.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Current licensure as a Physical Therapist in the state of New Jersey.
- Experience working with geriatric populations, preferably in home care or senior living settings.
- Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with flexibility to adapt to changing priorities and schedules.
