We are seeking a skilled and compassionate Physiotherapist to join our team in a newly developed frailty assessment unit. This is an exciting opportunity for a versatile postgraduate professional to work as part of a multi-professional team within the acute and community setting.

About the Role
  • The successful candidate will have experience working with people who have Frailty syndrome and enjoy working in a fast-paced, dynamic environment.
  • They will be responsible for supporting patients in their transition between acute and community sectors, providing seamless care and reducing the risk of loss in function, deconditioning, nutritional deficit, and deterioration in mental health.

Key Responsibilities:

  • Supervising students, junior members of staff, rehabilitation assistants, and therapy support staff.
  • Working alongside acute and community colleagues and services to create a joined-up rehabilitation pathway and positive experience for our population.
  • Managing a diverse caseload using clinical reasoning processes and negotiating multidisciplinary team working practices.

Salary Range: £43,141-£70,448 per annum.
