Nurse - Enhanced Intermediate Care Service

We are seeking a highly skilled and experienced Nurse to join our Enhanced Intermediate Care Service team in Bracknell. As a Senior Nurse, you will play a key role in providing high-quality care to patients in their own homes, promoting independence and self-care.
Key Responsibilities- Work as part of a multidisciplinary team to deliver intermediate care and physical rehabilitation services to patients in Bracknell Forest.
- Conduct assessments and develop care plans to support patients in achieving their goals and improving their quality of life.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Provide education and support to patients and their families to promote self-care and independence.
- Contribute to the development and implementation of care pathways and protocols to improve patient outcomes.
- Degree in Nursing and current NMC registration.
- Recent relevant clinical experience in community nursing or intermediate care.
- Valid driving license and access to a car for work purposes.
- Ability to work flexibly across service areas to enhance quality of service provision.
