About the Role

We are seeking a Relief Support Worker to join our team at LiveWest, providing support to customers living in the community who have severe and enduring mental health needs. As a Relief Support Worker, you will play a vital role in ensuring a positive outcome for our customers, supporting them to remain well and out of hospital.

Key Responsibilities

  • Provide person-centered support to customers, promoting independence and well-being
  • Work collaboratively with customers, families, and other professionals to develop and implement individualized support plans
  • Contribute to the development and maintenance of a safe and supportive living environment
  • Participate in regular team meetings and contribute to the development of the service

Requirements

  • Experience of working with vulnerable adults, preferably in a community care setting
  • Excellent communication and interpersonal skills
  • Ability to work flexibly, including evenings and weekends
  • Knowledge of relevant legislation and policies, including the Mental Health Act and the Care Act
