About Charles River Community HealthCharles River Community Health is a mission-driven organization committed to providing comprehensive medical, pharmacy, dental, behavioral health, optical, and vision services to underserved communities. We value timely access to care, collaboration with organizations for continuity of care, and community partnerships to meet patients' needs.
Responsibilities- Provide holistic, evidence-based medical care services for health center patients.
- Evaluate, treat, and care for patients, write prescriptions, provide preventive health care advice, and assess the psychological impact of illness.
- Refer patients for specialty services and provide documentation according to health center procedures.
- Participate in co-management of a panel with team-based PA or NP.
- Participate in clinical training activities at the health center.
- Contribute to the formation and evaluation of clinical protocols.
- Maintain a patient panel of a minimum of 1500 patients.
- Ensure continuity of care when patients utilize other healthcare facilities.
- Participate in staff meetings, including quality assurance and medical audits.
- Participate in paid 24-hour on-call medical coverage approximately 1-2 times per quarter.
- Work one evening (until 7pm) weekly; occasional Saturdays may be required.
- Collaborate with staff to meet ACO quality goals, DPH accreditation, and other regulatory requirements.
- Perform other related duties as assigned by the Medical Director.
- M.D. degree and completion of a residency in family medicine, internal medicine, or pediatrics required.
- Must be board-eligible with a plan to become board-certified or board-certified in your chosen specialty.
- Current Massachusetts medical and prescribing license and DEA required.
- Must apply for and be granted privileges at affiliated hospital(s).
- Understanding of ambulatory family practice healthcare program administration preferred.
- Experience with Patient Centered Medical Home model preferred.
- Knowledge of EMR and Microsoft Office systems required; familiarity with Epic desired.
- Ability to work out of either our Brighton or Waltham locations as needed.
- Fluency in Spanish and/or Portuguese a plus.
- Excellent communication skills, particularly with diverse cultures.
- Commitment to promoting diversity, equity, and inclusion.
- Participate in continuing medical education activities as required for licensure.
- Passion for serving underserved populations in diverse settings.
