Job Title: Band 6 Community Mental Health Practitioner

We are seeking a highly skilled and experienced Band 6 Community Mental Health Practitioner to join our team in Witney, Oxfordshire.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality care to patients with severe and enduring mental health diagnoses.

As a Band 6 Community Mental Health Practitioner, you will be responsible for coordinating care, assessing and planning treatment, and managing risk. You will work closely with patients, carers, and other healthcare professionals to deliver effective care.

The successful candidate will have extensive experience of working in community mental health care, with a strong understanding of clinical risk assessment and management.

Key Responsibilities
  • Coordinate care for patients with severe and enduring mental health diagnoses
  • Assess and plan treatment with service users
  • Manage risk, including use of Safeguarding processes
  • Work collaboratively with patients, carers, and other healthcare professionals
Requirements
  • Registered Professional Qualification (Mental Health Nurse / OT)
  • Evidence of Continuing Professional Development (CPD) portfolio
  • Experience of working within a multidisciplinary team
  • Relevant experience in community care and specific clinical expertise in mental health
