Job Title: Band 6 Community Mental Health Practitioner

We are seeking a highly skilled and experienced Band 6 Community Mental Health Practitioner to join our team at North and West AMHT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with severe and enduring mental health diagnoses.

About the Role

As a Band 6 Community Mental Health Practitioner, you will be responsible for coordinating care for patients with complex mental health needs. You will work closely with our multidisciplinary team to assess and plan treatment, manage risk, and provide interventions to patients and carers. You will also be required to travel around the county to offer interventions, so access to transport is essential.

Key Responsibilities
  • Coordinate care for patients with severe and enduring mental health diagnoses
  • Assess and plan treatment with service users
  • Manage risk, including use of Safeguarding processes
  • Provide interventions to patients and carers
  • Travel around the county to offer interventions
Requirements
  • Registered Professional Qualification (Mental Health Nurse / OT)
  • Evidence of Continuing Professional Development (CPD) portfolio
  • Experience of working within a multidisciplinary team
  • Relevant experience in community care and specific clinical expertise in mental health
  • Demonstrate an understanding of clinical risk assessment and management
  • Experience of supervising/managing staff
