As a Band 7 Senior Community Mental Health Practitioner, you will be responsible for assessing, planning, implementing, and evaluating care and treatment plans for individuals with mental health needs. Your key duties will include:
- Conducting comprehensive assessments and developing personalized care plans
- Providing evidence-based interventions and therapies to promote recovery and well-being
- Liaising with other healthcare professionals and agencies to ensure seamless care delivery
- Collaborating with the multidisciplinary team to achieve positive outcomes
To be successful in this role, you will need:
- NMC qualification as a registered nurse or allied health professional
- Experience of working in a mental health setting, ideally with a focus on community care
- 3 years post-qualification experience, with a strong understanding of mental health theories and models
