The Role:

We are looking for a skilled and empathetic Social Worker to join our CHC team, focusing on supporting individuals with complex needs through the discharge process. You will be responsible for conducting assessments, collaborating with multidisciplinary teams, and facilitating transitions to appropriate care arrangements.

Your Qualifications:

">

To be successful in this role, you should possess a Social Work qualification and be registered with Social Work England (SWE). In addition, you should have experience in adult social care, familiarity with the National Framework for NHS Continuing Healthcare, and a valid driver's licence.
