We are seeking a skilled Community Support Worker to join our Home First North Team at Cornwall Partnership NHS Foundation Trust.

The successful candidate will provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting. This will involve working with patients in their usual place of care, including residential homes and nursing homes, and being flexible in working patterns to meet the needs of the service.

The post holder will work under the guidance and clinical supervision of the locality team, including ward and community staff, and will be expected to complete the Ageing well competencies training programme. They will also actively participate in their own wider learning, including clinical supervision and peer supervision.

The main responsibilities of this role include:

  • Assisting patients to achieve their reablement goals using a range of specific activities, primarily on an individual basis, but also in groups, without direct supervision.
  • Supporting with personal care and promoting a return to normal function and activity as appropriate.
  • Encouraging the appropriate use of aids/equipment and reinforcing self-care wherever possible.

We are an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Requirements

The ideal candidate will have:

  • NVQ 3 or equivalent experience in a range of subjects relevant to rehabilitation/health and social care.
  • Previous experience of providing care/reablement in a health or social care setting.
  • A valid current UK driving licence.
