Floating Support Specialist - Caerphilly

Platfform is seeking a highly skilled Floating Support Specialist to join our team in Caerphilly. As a key member of our floating support team, you will work closely with individuals experiencing challenges with their mental health, providing guidance and support to help them achieve their goals.

The ideal candidate will have a strong understanding of mental health issues and be able to provide empathetic and non-judgmental support to service users. You will work collaboratively with colleagues and external agencies to develop tailored support plans that meet the unique needs of each individual.

Key responsibilities include:

  • Conducting assessments and developing support plans with service users
  • Providing one-to-one and group support sessions
  • Working closely with colleagues and external agencies to ensure seamless delivery of services

Salary: £26,000 - £32,000 per annum (dependent on experience)
