About the Role
We are seeking a dedicated and compassionate Support Worker to join our team at St Mungo's, working with individuals experiencing homelessness in Hammersmith & Fulham. As a Support Worker, you will play a vital role in providing personalized support and guidance to our clients, helping them to achieve their goals and improve their overall well-being.
Key Responsibilities
- Work closely with clients to identify their strengths, aspirations, and personal goals, and develop a tailored support plan to help them achieve their objectives.
- Provide practical and emotional support to clients, including assistance with daily living tasks, benefits, and housing-related issues.
- Collaborate with internal teams, including housing, health, and social care services, to ensure a comprehensive and coordinated approach to client support.
- Conduct regular reviews and assessments to monitor client progress, identify areas for improvement, and adjust support plans as needed.
- Develop and maintain strong relationships with clients, their families, and other stakeholders to ensure a supportive and inclusive environment.
Requirements
- Experience working with vulnerable populations, particularly those experiencing homelessness or housing instability.
- Strong interpersonal and communication skills, with the ability to build trust and rapport with clients from diverse backgrounds.
- Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the changing needs of clients and services.
- Knowledge of local services, resources, and support networks, with a commitment to connecting clients with relevant services and opportunities.
- Passion for delivering high-quality, person-centered support, with a focus on empowering clients to achieve their goals and improve their overall well-being.
