We are seeking an experienced Community District Nurse to join our team at Dorset HealthCare University NHS Foundation Trust.

About the Role

This is an exciting opportunity for a skilled and dedicated professional to work as a registered nurse, providing evidence-based, high-quality care for patients in their own homes. You will be working with community nursing teams, assessing patient needs, planning, delivering, and evaluating care tailored to individual requirements.

The successful candidate will have experience working in community settings, preferably in a similar role, with excellent communication and interpersonal skills. A strong knowledge of wound care and palliative care principles is also essential.

Main Responsibilities
  1. Deliver holistic nursing care to patients in their own homes, focusing on prevention, early intervention, and timely treatment.
  2. Conduct thorough assessments, develop care plans, and coordinate with healthcare professionals to ensure seamless delivery of services.
  3. Build relationships with patients, carers, and other stakeholders to promote understanding and cooperation.
  4. Continuously evaluate and improve care delivery, adhering to best practices and evidence-based guidelines.
