Salary

The salary for this position is estimated to be around £40,000 - £60,000 per annum, depending on experience.

Job Description

As a District Nurse with Dorset HealthCare University NHS Foundation Trust, you will provide high-quality, evidence-based care to patients in their own homes.

You will work closely with other healthcare professionals, including GPs, consultants, hospital nurses, and social workers, to assess patient needs and develop individualized care plans.

Your key responsibilities will include:

  • Conducting comprehensive assessments of patients' physical, emotional, and social needs;
  • Developing and implementing individualized care plans that address patients' specific needs;
  • Providing high-quality, evidence-based care to patients in their own homes;
  • Liaising with other healthcare professionals to ensure seamless care coordination;
  • Working collaboratively with carers, family members, and other stakeholders to support patients' needs.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A valid registration with the Nursing and Midwifery Council (NMC);
  • Experience working as a District Nurse or in a similar community care setting;
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals;
  • Ability to work independently and as part of a team, with a flexible approach to workload and priorities;
  • Good knowledge of relevant policies, procedures, and guidelines, including those related to patient safety and confidentiality.
