Job Summary

We are seeking a highly motivated and compassionate Support Worker to join our Community Services team in West Kent. As a Support Worker, you will play a vital role in supporting individuals with learning disabilities to access and participate in their local community.

Key Responsibilities

  • Provide person-centered support to individuals with learning disabilities, promoting their social inclusion and rights.
  • Work collaboratively with colleagues and senior staff to develop and implement individualized support plans.
  • Assist individuals with daily living tasks, such as personal care and medication management.
  • Support individuals to access community resources and services, including education and employment opportunities.
  • Develop and maintain relationships with community partners and stakeholders to ensure seamless support services.
  • Participate in regular team meetings and contribute to the development of best practices in support work.

Requirements

  • Relevant experience in care and support work, including experience working with individuals with learning disabilities.
  • Hold a Diploma Level 2 in Health and Social Care or equivalent qualification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.

Working Arrangements

This is a 22.5 hour per week post, with set days and hours. However, these may be subject to change to support business needs.
