About the Role
We are seeking an experienced Advanced Nurse Prescriber to join our team as a Clinical Lead for Primary Care Services. This is an exciting opportunity to work in a forward-thinking and innovative GP group, providing primary care services to 49,000 patients across 5 local sites in Gosport.
The successful candidate will have excellent clinical, organisational & communication skills, with the ability to work autonomously and provide strong leadership to the team. They will be responsible for supporting our on-going initiatives, including the Gosport Home Visiting Service and our Same Day Access Service.
The role involves holding own clinics, demonstrating critical thinking in clinical decision-making, and working collaboratively with the general practice team and extended primary care team to meet the needs of patients. The post holder will also develop new and innovative concepts, models, methods, and practices to deliver new and improved primary care services to meet the needs of the practice population.
Main Responsibilities
As a Clinical Lead, you will be responsible for:
- Providing safe, effective, and caring services to patients, including initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care;
- Demonstrating safe and expert care, including assessment and diagnostic skills, for patients within the general practice;
- Using a medical model to assess, diagnose, and prescribe for acute medical conditions, and a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team;
- Participating in and influencing efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention through proactive provision of care in or as close to the patients' own home as possible;
- Providing strong leadership to the team, including day-to-day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well-led' culture.

Southern Health NHS Foundation Trust is one of the largest NHS Foundation Trusts in the UK, specialising in mental health and learning disabilities, as well as offering physical health community-based services. We have a workforce of over 7,000 and operate across more than 200 sites across Hampshire. Our new organisation, Hampshire and Isle of Wight NHS Foundation Trust, is set to launch in July 2024, bringing together Southern Health and Solent Trusts to provide integrated community, mental health, and learning disability services.

What We Offer
