About the Role

We are seeking a highly motivated and experienced Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community.

Key Responsibilities
  • Creating personalized support plans with customers to help them achieve their goals and aspirations
  • Providing daily living support, including budgeting, tenancy management, and independent living skills
  • Conducting risk assessments, support planning, and regular reviews to ensure customers receive the best possible support
  • Supporting and signposting customers to access external services to promote their independence
  • Working collaboratively with colleagues to ensure seamless support and care for customers
Requirements
  • Passion for supporting vulnerable individuals and promoting independence
  • Experience of creating person-centered support plans and supporting colleagues in working to these plans
  • Experience of coordinating and assessing customer referrals
  • Ability to work independently, remain calm under pressure, and have a resilient approach
  • Valid driver's license and access to a vehicle insured for business purposes
