About the Role

We are seeking a highly skilled and dedicated Health Visitor Specialist Community Public Health Nurse to join our team at South Tyneside and Sunderland NHS Foundation Trust. As a key member of our public health team, you will play a vital role in improving the health and wellbeing of children and families in our community.

Key Responsibilities
  • Work unsupervised as a health visitor with a caseload responsibility to influence the health and wellbeing of children and families.
  • Lead the universal national Healthy Child Programme through home visiting, clinics, and multi-agency settings.
  • Work with a wide range of partner agencies to support children and families in need, looked after children, and children where there are child protection concerns.
  • Be the Lead Professional for assessment, early identification, and care planning for vulnerable families, children with additional needs, and the implementation of targeted specialist programmes of care.
  • Work in collaboration with local statutory, voluntary, and mainstream services to ensure that children and families can access other health and support services.
  • Be responsible and accountable for the delivery of safe, effective, and compassionate care.
Requirements
  • Registered Nurse or Midwife qualification.
  • Specialist Community Public Health Practitioner-Health Visiting qualification.
  • Experience of working in a community setting, public health role, and home visiting.
  • Experience of working in a multi-agency way.
  • Ability to organise and prioritise workload.
  • Works to agreed standards and protocols.
  • Excellent communication and interpersonal skills to manage discussions with families and other agencies.
  • Able to work effectively as part of a team.
  • Able to carry out clinical and social assessment to make the case for referral resources for families.
  • Able to work effectively with a range of other multi-agency professionals to ensure best outcomes for children.
  • IT skills to enable the production of reports, communicate via email, receive reports, and communication.
  • Able to carry out audit of workload as required.
  • Resilience, self-awareness, non-judgmental, driven to improve outcomes for children.
  • Knowledge of current policy, legislation relating to children, families, and public health.
  • Willing to undertake further training related to support role development.
