Position Overview

An exceptional opportunity has emerged for a passionate and dedicated Physiotherapist to become a vital member of our Recovery at Home Team.

This role is both unique and fulfilling, enabling you to collaborate with, guide, and support colleagues during a transformative period as our team emphasizes the execution of Urgent Community Response and Virtual Wards.

You will be integrated within Directorate One of Community Services, receiving support from senior professionals who take pride in the services we provide to our community.

Key Responsibilities

The team you will join comprises a diverse multidisciplinary team (MDT) that includes nurse practitioners, occupational therapists, physiotherapists, healthcare assistants, therapy assistants/exercise practitioners, and administrative personnel. This collaborative approach, alongside local and voluntary sector partners, is a significant asset to our team, promoting patient-centered and integrated care.

The team provides both planned and emergency care to individuals in their homes, aiming to prevent hospital admissions and assist those recently discharged from hospital who require enhanced intervention and short-term rehabilitation support for their physical and functional health needs. The service also includes rotation into our bed-based reablement unit to facilitate patients' transition from hospital to home, supporting their independence.

This service operates in a fast-paced environment, and you will be expected to engage with a wide array of patients presenting with various conditions while providing clinical support to the team.

Job Duties

• Deliver care across services that provide Urgent Community Response, Discharge to Assess, and short-term rehabilitation.

• Offer care to individuals in their homes to avert hospital admissions/readmissions through rapid patient assessments in the community.

• Conduct comprehensive holistic assessments of patients in both planned and unplanned scenarios, prioritizing clinical needs, initiating investigations, determining differential diagnoses, and implementing care plans based on evidence-based guidelines.

• Maintain agreed standards of care to uphold the integrity and dignity of all patients in environments conducive to delivering care in their usual or temporary residences, including bed-based rehabilitation.

• Perform triage within the service to determine clinical priority of calls, coordinating team responses to referrals.

• Collaborate with multidisciplinary teams to ensure practices are efficient, effective, and evidence-based.

• Develop your professional practice scope to support the team in meeting service requirements in line with professional guidelines.

• Provide expert knowledge to all service users.

Qualifications and Skills

Essential

  • Car driver with access to a vehicle for work purposes.
  • Registered with the HCPC.
  • Degree in relevant field.
  • Willingness to pursue a teaching qualification as a practice educator.
