Job Overview

The Ambulatory Outreach Nurse will play a crucial role in delivering high-quality patient care in the community. As a key member of the Ambulatory Outreach Team, you will work closely with patients, families, and healthcare professionals to provide holistic care and support.

Main Responsibilities
  • Conduct comprehensive clinical assessments and develop individualized care plans for patients in their homes.
  • Collaborate with multidisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
  • Provide education and support to patients and families on self-management and disease prevention.
  • Contribute to the development and implementation of innovative care pathways and services.
  • Work autonomously and as part of a team to achieve shared goals and objectives.
