Clinical Support Specialist

Buckinghamshire Council is seeking a highly skilled Clinical Support Specialist to join their Hospital Discharge South and Central Team based at Amersham Hospital.
About the RoleThis exciting opportunity enables you to utilise your expertise in healthcare to provide goal-based support for individuals, promoting independence and improving wellbeing. You will work collaboratively with colleagues to ensure seamless care delivery, focusing on proactive personalised approaches and timely responses to client needs.
The hospital discharge teams operate a 7-day service, requiring flexible working arrangements, including occasional weekends. As a Clinical Support Specialist, you will bring your in-depth knowledge of legislation and statutory guidance to contribute to the team's success.
Key Responsibilities- Deliver high-quality care and support, prioritising individualised goals and outcomes.
- Collaborate with colleagues to identify and mitigate risks, ensuring the safety and wellbeing of clients.
- Provide education and training to support colleagues within the team.
- Develop and maintain effective relationships with internal stakeholders and external partners.
- Advanced Practitioner qualification or equivalent experience.
- In-depth knowledge of legislation and statutory guidance related to healthcare.
- Proven ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- A competitive salary of £43,819 - £49,337 per annum, inclusive of Market Factor.
- 'Golden Hello' lump-sum payment of £2500 paid on successful completion of your probationary period.
- Up to 25 days' annual leave (plus bank holidays) rising to 30 days (plus bank holidays) after more than 5 years' service.
- Relocation allowance up to £8,000 (subject to eligibility).
