**Practice Nurse Role Overview**

Celsus Group Limited is seeking an experienced Clinical Care Professional to join our team in Chesham. This part-time role will cover essential nursing services over two or three days.

The ideal candidate will possess strong clinical skills and be able to manage chronic conditions, perform wound care, vaccinations, and health checks. Additionally, they will support patients with minor illnesses and injuries, contribute to the development of care plans and treatments, and collaborate with the multidisciplinary team to ensure patient-centered care.

About the Role:

  • Manage Chronic Conditions: The successful candidate will be responsible for managing diabetes, respiratory, and cardiovascular conditions, ensuring that patients receive high-quality care and support.
  • Wound Care and Vaccinations: They will perform wound care and vaccinations, as well as conduct health checks to identify potential issues early on.
  • Minor Illnesses and Injuries: The Clinical Care Professional will provide support to patients with minor illnesses and injuries, using their clinical knowledge and skills to provide effective treatment and care.
  • Care Plans and Treatments: They will contribute to the development of care plans and treatments, working closely with the multidisciplinary team to ensure that patients receive comprehensive care.
  • Patient-Centered Care: The successful candidate will collaborate with the team to deliver patient-centered care, prioritizing the needs and preferences of each individual.

Requirements:

  • Strong clinical skills and experience in managing chronic conditions
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Knowledge of relevant healthcare policies and procedures

What We Offer:

  • A competitive salary range: £35,000 - £42,000 per annum, depending on experience
