About the Role

We are seeking a highly motivated and experienced Team Leader to join our team at Active Care Group. As a Team Leader, you will be responsible for leading a team of support workers and ensuring the delivery of high-quality care and support to our service users.

Key Responsibilities
  • Lead a team of support workers to deliver care and support to service users
  • Develop and implement care plans in conjunction with the clinical team
  • Ensure the delivery of high-quality care and support to service users
  • Monitor and evaluate the effectiveness of care and support plans
  • Provide supervision and support to support workers
  • Collaborate with the clinical team to ensure the delivery of high-quality care and support
Requirements
  • Level 3 NVQ or QCF Diploma in Health and Social Care or equivalent
  • Experience of working in a care and support setting
  • Strong leadership and management skills
  • Excellent communication and interpersonal skills
  • Ability to work as part of a team
  • Ability to work flexibly and respond to changing circumstances
