Primary Care Support Specialist

The Care Coordinator will be responsible for providing coordination and navigation of care and support across health and care services. This will involve working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals.
You will also assist with the coordination of home visits, help patients manage their needs through answering queries and making appointments, and ensure they have good quality information to make choices about their care.
In addition, you will promote access to self-management education courses, peer support, health coaching, and other interventions that support people in their health and wellbeing.
