Job Title: Primary Care Network Specialist

K & W Healthcare Ltd. invites applications for a highly skilled Primary Care Network Specialist to join our team as a Full-time employee.

About the Role:

  • To lead the integration of community services to deliver seamless patient care across primary care networks.
  • To develop and implement strategies to improve productivity and capacity within primary care settings.
  • To collaborate with healthcare professionals to enhance patient outcomes and experience.

Key Responsibilities:

  • To coordinate the development and implementation of integrated care plans.
  • To facilitate collaboration between primary care network leads, community, and hospital teams.
  • To monitor and evaluate the effectiveness of integrated care initiatives.

Requirements:

  • A strong understanding of primary care network structures and operations.
  • Excellent leadership and project management skills.
  • The ability to communicate effectively with diverse stakeholders.

Working Arrangements:

  • This is a full-time position, working 8 hours per day, Monday to Friday.
  • The successful candidate will be expected to commute reliably to designated locations within the Brent area.

Estimated Salary Range: £40,000 - £55,000 per annum, dependent on experience.
