Vox Network Consultants is seeking an experienced Social Worker to join a London Borough Council's Adult Transitions Team on a locum basis. As a key member of the team, you will play a vital role in supporting young adults with learning disabilities under the Care Act.
Key Responsibilities- Conducting home visits and assessments to identify client needs
- Developing strong relationships with clients and their families
- Maintaining accurate records and reporting progress
- Working collaboratively with other professionals to achieve best outcomes

Your Background- Bachelor's Degree in Social Work
- ASYE completion
- At least 2 years of post-qualifying experience as a Social Worker
- Registration with Social Work England
