The estimated salary for this position is £38,890 - £44,997 per annum.

Job Description
We have an exciting opportunity for a highly motivated and enthusiastic clinician to join our Older Adult Community Treatment Team. As a Band 6 Community Practitioner, you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions, focusing on care homes. You will work in a highly supported environment and be provided with training opportunities to develop your knowledge and skills. Your strong commitment to delivering high-quality, safe patient care is essential.

Main Duties
• Responsible for assessing and managing casework of service users with substantial and complex needs
• Develop formulation-focused care plans using a psychosocial approach, ensuring timely reviews to monitor progress and adapt care plans as necessary
• Provide compassionate care based on empathy, kindness, respect, and dignity
• Deliver clinical and psychologically based interventions
• Provide specialist advice to service users, carers, staff, statutory agencies, third sector partner agencies, and the public
• Provide clinical supervision to staff as delegated by the Clinical Lead and Pathway Manager

Person Specification
• Professional qualification and current professional registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist
• Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post
• Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection
• 12-24 months of band 5 qualified nurse experience dependent on service need
• Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives
• Risk Assessment and Risk Management/Safety Planning skills
