Job Title: Care Coordinator Specialist

Salary Range: Up to £29,000 per annum (pro rata)

About the Role

This is a remote Client/Patient Coordinator position that requires strong communication skills and the ability to work independently.

  1. We are seeking an individual who can effectively coordinate clients and referrals.
  2. Prioritization and management of referral waiting lists will be key responsibilities.
  3. The successful candidate will process sensitive client information and liaise with social workers as needed.
  4. Regular communication with clients and third parties via email and phone will be essential.
Key Requirements
  • Strong Communication Skills: The ability to provide support over phone and email is crucial.
  • Data Management: Proficiency in working with data and computer literacy is required.
  • Personable Nature: Comfortable interacting with vulnerable individuals on occasion.
  • Decision Making: Ability to prioritize tasks and make informed decisions.
  • Advocacy Experience: Although not essential, any relevant experience in advocacy or social care is beneficial.
