Care Coordinator Specialist

We are seeking a highly organized and dynamic Care Coordinator Specialist to join our fast-paced Bridging Project team. This pivotal role involves coordinating high-quality home care services to support individuals transitioning between hospital and home, or within the community.
The ideal candidate will have proven experience in domiciliary care coordination or a similar role, with a strong understanding of care planning, risk assessments, and CQC requirements.
Key Responsibilities:
- Plan, organize, and manage care schedules for service users, ensuring appropriate allocation of carers.
- Respond promptly to changing care needs, making adjustments to schedules as required.
- Work collaboratively with healthcare professionals to ensure smooth care transitions.
In this role, you will lead and support the care team, providing guidance and supervision to maintain high standards. You will also act as the main point of contact for clients and their families, addressing concerns or issues promptly.
We offer a competitive salary up to £30K pa depending on experience, plus commission on GP. Ongoing training and professional development opportunities are also available.
