Job Description

Cornwall Partnership NHS Foundation Trust is an NHS community and mental health provider trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

This is a challenging yet rewarding opportunity to work as part of our community support team, providing assessments and developing positive behavioral approaches for individuals at risk of hospital admission or placement breakdown. You will work collaboratively with individuals, families, and care providers to achieve patient-centered outcomes and improve quality of life.

The ideal candidate will possess strong clinical skills, experience working with people with learning disabilities, and a commitment to evidence-based practice. You will be able to use reflective practice and clinical supervision to provide high-quality specialist assessments and interventions.
