We are seeking an experienced Home Care Provider to join our team at Premier Community, a leading provider of home care services in the UK.
This rewarding role involves working closely with clients to understand their needs and preferences, providing personalized care and support to enable them to live safely and independently in their own homes.
Your key responsibilities will include:
- Conducting thorough assessments of clients' needs and developing personalized care plans.
- Delivering high-quality care and support, including assistance with daily living tasks, meal preparation, and medication administration.
- Maintaining accurate records of client information and care activities using our digital care planning system.
- Collaborating with colleagues and other healthcare professionals to ensure seamless care delivery.
To excel in this role, you'll need:
- A minimum of 12 months' experience in a similar role or related field.
- Excellent communication and interpersonal skills.
- A commitment to delivering high-quality care and supporting individuals to achieve their goals.
