About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Community Response Teams as a Community Health Care Specialist. Based in Leamington and Stratford, this exciting opportunity offers a dynamic individual the chance to work autonomously within a team environment, providing holistic care to patients in their own homes.

Key Responsibilities

  • ">
  • Assessing patient needs and developing personalized care plans to ensure optimal health outcomes;">
  • Working collaboratively with multidisciplinary teams, including GPs, social services, and urgent response, to deliver seamless care across the locality;">
  • Providing skilled clinical nursing practices, including people moving and handling, and operating as a lone worker when necessary;">
  • Developing and maintaining effective communication networks with service users, healthcare professionals, and other statutory and voluntary organizations;">
  • Maintaining accurate records and adhering to NMC policy and Data Protection guidelines;">

What We Offer

We offer a competitive salary of £34,000 - £44,503 per annum, depending on experience. This role also comes with excellent benefits, including mileage reimbursement for work-related travel and opportunities for professional development and growth within the trust.

Requirements

To be successful in this role, you must possess a valid full NMC registration, a driving license, and access to your own car for work purposes. You should also be willing to work flexible hours, including Bank Holidays and weekends, and participate in cover of the evening service if required.
