Job Title: On Street Support- Housing Solutions

Location: Havering

Hours: 9am-5:30pm

Term: Temporary until Feb 2024 (with the possibility of extension)

Salary: 17.54 PAYE hour

We are seeking a dedicated and passionate individual to join our team as an On Street Support- Housing Solutions. As a key member of our team, you will play a vital role in helping individuals with multiple and complex needs prevent a return to rough sleeping.

Key Responsibilities:

  • Develop and implement support plans to ensure clients sustain their quality of life
  • Engage with various support agencies to promote personal independence and well-being
  • Act as the main point of contact for casework within the accommodation, coordinating solutions for drug and alcohol, domestic violence, and mental health issues

Requirements:

  • Demonstrable experience in case planning and complex case management
  • Sensitivity to the diverse needs of individuals from a range of backgrounds
  • Strong communication skills, both written and verbal
  • Knowledge of local social and health care services and the ability to navigate them effectively
