Integrated Health and Social Care Coordinator

Main Responsibilities:
- To support vulnerable adults to sustain or improve their quality of life and well-being through person-centred planning and coordination of community-based resources.
- Work collaboratively with multi-disciplinary teams, including the voluntary sector, to promote independence and personalisation.
- To act as a main point of contact for residents and their unpaid carers, providing support to maximise independence and utilise community resources effectively.
Requirements:
- A minimum of 2 years' experience in social work, with QSW status and registration with Social Work England.
- Right to work in the UK.
Benefits: A competitive salary of £38,890 - £44,688 per annum (dependent on experience), plus benefits
