Job Summary

EPUT is seeking a motivated and dedicated Registered Mental Health Nurse (RMN) to join our Specialist Dementia and Frailty Team as a Band 5 Student District Nurse. As a key member of our team, you will provide high-quality, evidence-based care to patients with dementia and frailty, working closely with the Advanced Nurse Practitioner to assess, plan, implement, and evaluate packages of care.

Key Responsibilities
  • Assess and review care plans for patients with dementia and frailty, ensuring that care is delivered in line with Trust policies and professional codes of conduct.
  • Work collaboratively with the Advanced Nurse Practitioner to develop and implement care plans that meet the individual needs of patients.
  • Supervise and manage junior team members, providing guidance and support to ensure the delivery of high-quality care.
  • Conduct risk assessments and manage risks associated with patient care, ensuring that patients receive safe and effective care.
  • Participate in the development and implementation of Trust policies and procedures, ensuring that they are aligned with the Trust's values and objectives.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Current driving license and access to a car for work purposes.
  • Ability to work flexibly, including evenings and weekends, as required.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Knowledge of health legislation, principles of CPA, and Care Management.
