Job Title: Registered Mental Health Nurse

We are seeking a highly skilled and dedicated Registered Mental Health Nurse to join our team at the Transformation Unit.

About the Role

The successful candidate will be responsible for providing high-quality, evidence-based care to patients with dementia and frailty, working closely with the Advanced Nurse Practitioner to assess, plan, implement, and evaluate packages of care.

As a member of the Mid-Dementia and Frailty Pathway team, you will have the opportunity to work in a dynamic and supportive environment, collaborating with colleagues to deliver excellent patient outcomes.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of care in line with mental health care clusters.
  • Develop and implement individualized care plans, ensuring patient-centered and holistic approaches.
  • Supervise and manage junior team members, providing guidance and support to ensure high-quality care delivery.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and effective communication.
  • Participate in performance management and supervision of lower-banded staff, promoting professional growth and development.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Experience in performance management and supervision of lower-banded staff.
  • Understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
  • Full UK driving license and access to a car for work purposes.
