We are seeking a highly skilled Clinical Pharmacist to join our team at Island Health Primary Care LTD. As a Clinical Pharmacist, you will play a vital role in providing expert medicines advice and support to our general practice staff, patients, and community pharmacy teams.

Key Responsibilities
  • Provide clinical leadership on medicines optimisation and quality improvement initiatives
  • Develop and implement strategies to improve patient safety and reduce medication-related harm
  • Collaborate with community and hospital pharmacy teams to ensure seamless care transitions and optimal patient outcomes
  • Conduct structured medication reviews and provide recommendations for medicine improvement
  • Develop and maintain effective relationships with general practice staff, patients, and community pharmacy teams
Requirements
  • Masters degree in pharmacy (MPharm) or equivalent
  • Completed 18-month CPPE pathway or equivalent
  • Independent prescriber status
  • Prior GP/clinical pharmacy experience
  • Specialist in-depth knowledge for multiple chronic disease areas
