Case Management Specialist

Job Overview:
This exciting opportunity is for a highly motivated and experienced Case Management Specialist to join our Family Support team in Stoke-on-Trent. The successful candidate will work closely with colleagues to develop and implement effective case management strategies, ensuring the best possible outcomes for children and families.
Key Responsibilities:
- To conduct thorough assessments and create personalized plans for families in need.
- To coordinate and facilitate meetings with key stakeholders, including family members, carers, and professionals.
- To maintain accurate records and report progress to senior managers.
Requirements:
- A degree in Social Work or equivalent qualification.
- Proven experience in case management and family support.
- Excellent communication and interpersonal skills.
