We are seeking an experienced Intensive Case Manager to join our Dementia Intensive Support Team in Suffolk, UK.

About the Role

This is a challenging and rewarding opportunity for a skilled professional to work with individuals with dementia and their carers, providing intensive support and guidance to help them live independently in their communities.

As an Intensive Case Manager, you will be responsible for managing a caseload of clients, conducting initial assessments, engaging with clients and their families, and collaborating with other professionals to ensure effective care and support.

Main Responsibilities
  • Manage and hold a caseload of clients with dementia and their carers
  • Conduct initial assessments, including risk assessments, to identify client needs
  • Develop and implement individualized care plans to support clients in living independently
  • Liaise with statutory and voluntary sector organizations to access resources and services
Requirements
  • Nursing degree or equivalent
  • Experience working within a multidisciplinary team
  • Familiarity with risk assessment and management
  • Ability to travel independently
Working for Us

We offer a competitive salary range of £38,000 - £50,000 per annum, depending on experience.
