About the Job



As a Community Nurse at Kent Community Health NHS Foundation Trust, you will play a vital role in supporting the nursing needs of housebound patients living in and around Thanet. You will enable and empower patients to self-care where possible, so they can stay safe and well at home or in their chosen environment.



Our team provides valuable care, supporting people to manage their long-term conditions and maximise their independence, which can prevent admission into hospital or enable a quicker discharge if they are admitted. You will work closely with our multidisciplinary services, managing patients with complex needs in the community.



Key Responsibilities



This role will involve:




  • Providing wound management, palliative and end of life care, and symptom control services to patients in their homes;
  • Conducting assessments and developing care plans to ensure patients receive the right support and services;
  • Working collaboratively with other healthcare professionals to deliver high-quality care and improve patient outcomes;
  • Supporting patients to manage their long-term conditions and maintain their independence;
  • Contributing to the development of local and national health agendas.

Person Specification



To be successful in this role, you will need to:




  • Be a registered healthcare professional with a relevant professional body;
  • Have experience of recent community or acute healthcare gained through education or work experience placement or previous employment;
  • Have knowledge of up-to-date clinical practice and professional issues in the NHS;
  • Have sound communication and interpersonal skills, with the ability to organise your own work and use your own initiative.
