Key Responsibilities- Develop and implement personalized plans to support clients in achieving their recovery goals
- Conduct assessments and risk assessments to identify client needs and develop strategies for support
- Work collaboratively with other social care providers to ensure comprehensive support for clients
- Maintain accurate and timely records of client interactions and progress
- Participate in ongoing training and professional development to enhance skills and knowledge
- ONC/NVQ level 3 or equivalent qualification
- Experience working in a health, social care, or related setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a team and build strong relationships with clients and colleagues
- Resilience and ability to prioritize tasks under pressure
