Health Visitor Specialist

Join us as a Health Visitor Specialist, where you will be working with the Healthy Family Team in central Lancashire. This position offers flexibility and the opportunity to work with a multi-disciplinary team, including Health Visitors, School Nurses, and other colleagues.
About the RoleYou will be responsible for delivering the Healthy Child Programme, using evidence-based practice to develop and maintain high-quality care standards. This includes assessing health needs, developing individualized care plans, and identifying unmet needs. As a Bank Health Visitor, you will have access to self-booking and out-of-hours enhancements, as well as opportunities for professional development and continuing education.
Key Responsibilities- Deliver the Healthy Child Programme, including assessment of health needs, development, implementation, and evaluation of packages of care.
- Use evidence-based practice to develop and maintain high-quality care standards.
- Assess health, growth, and development in the 0-5 age range (0-19 Pathway).
- Identify and assess unmet health needs and use the Early Help Assessment Tool when necessary.
To be successful in this role, you will need to have a SCPHN Health Visitor qualification, a valid UK driving license, and access to a car for work purposes. You should also have a passion for ensuring that children in Lancashire have the best possible start in life.
